We have each of our clients fill out a consent form for the following services they are receiving, for privacy and basic information purposes. These client forms are confidential and subject to change.
Privacy & Confidentiality:
All of our wonderful clients are treated equally and privacy is very important, from our environment to confidentiality with personal information.
Upon arrival, please notify us of any medical condition you may have or be concerned about. We have the ability to provide you with ingredients lists for all of our products, to ensure we keep you healthy and happy! Please notify your doctor if you have any concerns before or after your appointment, and do not hesitate to contact us as well. For medical reasons, if you are pregnant, you can tan after your first trimester. Please fill the portion about pregnancy on your consent form, or notify us of any changes in your health for your consent and medical information.
We value your feedback! Please let us know if you enjoy our services, and refer your friends and family! If you have any concerns, complaints, issues, or what have you... feel free to let us know at any time. This is very important to us in insuring you get the best Faux Finish experience!
Please allow us 24 - 48 hours to get back with you for an appointment request or proposal. Every service that we offer has its own booking recommendations, please look over that in our treatments section.
We recommend our new clients and body wrap clients arrive 10-15 minutes early to insure paperwork is completed and extra time to ensure all of your questions and concerns are answered.
Late Arrival or Cancellation:
If you are going to be more than 5 minutes late, please let us know. We book our appointments in a timely fashion, and we can make adjustments and changes accordingly to make sure you get taken care of. If you are 10 mins - 15 mins. late we may have to reschedule. Please give us a 24 hours notice upon cancellation, we appreciate that!
We allow them, but let us know in advance.
Minimum Age for Spa & Tanning:
There is no minimum age requirement for our services, but under the age of 18 requires consent from your parent or guardian. They will sign and date the bottom portion of your consent form, upon arrival at very first appointment. If you would like this form in advance, just contact us and we can send it to you. You will be able to bring this in with you if the parent or gaurdian cannot be with you.
We accept all forms of payment. Cash, Personal or Business Check, Credit or Debit through Squareup. We can also take advance payment over the phone with a credit or debit card and can send you a receipt immediatly. All visits or credits are good within 1 year from purchase date.
All forms of payment are accepted, and this deposit will credit your account and invoice immediatly. Deposit is non-refundable if you cancel your reservations. $50 fee will be charge for bridal makeup & hair services as well as photography. This is charge upon booking and will credit your balance. If you no show or cancel within 1 month this is non-refundable.
Refund or Exchanges:
No refunds will be given, unless prior authorization and arrangements by Faux Finish. You will not be able to return our retail products, but can exchange them if necessary to fit your needs. You will not be able to receive a refund on any services already performed, but we can try and fix the problem! Groupon & Living Social voucher refund must go through merchant directly.
A gratuity for our artistry and exceptional service is always your choice. We accept and appreciate gratuities, but they are not required or included.
Give the gift of beauty with a Faux Finish Gift Certificate. It can be used for services, retail products, and cannot be redeemed for cash. Available in any denomination. Expires 1 year from purchase date.
Our prices are subject to change without notice. Specials and discounts cannot be combined with any other offer, unless specified otherwise.
Groupon or Living Social:
Vouchers are only good until your voucher expiration date, then we will credit the new full value price and you are responsible for the difference. These visits exclude Holidays & Weekends!
